Salient features about Breast Reduction surgery

  • Breast Reduction surgery is a procedure that involves 3 main processes- to reduce the volume of the breast tissue, reposition the nipple-areola complex (NAC) to the ideal level and recontour the breast envelope to improve the breast shape.
  • 300gm to 1500gm breast tissue can be removed from each breast which will significantly reduce the weight on the chest.
  • Breast reduction surgery is usually a hospital-based procedure and will need a recovery time of about 3 weeks.
  • Breastfeeding may not be possible after large breast reductions as a very small volume of the functional breast is left behind.

Breast Feeding After Surgery

    30% to 70% of the breast tissue can be removed as a part of the breast reduction procedure and this depends on the volume and weight of the breast, the patient’s height, weight and body shape, and her requirements. The milk-carrying ducts in the residual breast tissue are not disrupted by the surgery. This means that the breastfeeding potential in young patients is maintained to a large extent. Many young women with large breasts tend to be poor breastfeeders. This would continue after the surgery too. Those who are able to breastfeed after Reduction Mammoplasty, may not be able to nourish their children completely by breastfeeding. Out of the various techniques utilized for breast reduction surgery, the short scar technique removes the least amount of tissue with minimal scars and also retains the function.

Breast Reduction and Breast Cancer

    During the surgery, a notable portion of the breast tissue is removed. This also aids in a reduction in the chances for breast cancer after breast reduction surgery. The removed tissue is sent for histopathology tests as a part of cancer screening. Regular screening and checkups should be continued after the surgery, as the cancer risk is not nullified. The mammography findings will be altered after the surgery due to the scarring and calcium deposits. Cancer detection rates are not significantly modified by a reduction mammoplasty procedure if regular screening using physical examination and mammography are done.

Breast Reduction: Procedure

    Pre-operative planning for the surgery is meticulously done with the patient sitting up/ standing. The skin and breast tissue to be removed in the procedure and the position of the nipples are pre-marked. The procedure is performed under general anesthesia. The surgery may take about 3 - 4 hours to finish depending on the amount of breast reduction and the technique used. The technique employed will depend on the extent of the breast to be reduced and the volume of skin sagging that needs to be corrected. Liposuction may be used as an additional procedure in some cases when there are easily available significant fat deposits. The vertical short scar technique is the preferred method in our center. Very large Reduction Mammoplasties may require inferior pedicle reduction which will leave an anchor shaped scar. We prefer to perform Reduction Mammoplasties as a hospital-based surgery.

After Breast Reduction Surgery

  • A fluffy dressing on the breasts will be placed after the surgery. You can start walking about within a few hours of the surgery.
  • Tube drains inserted during the surgery to remove fluid collection within the reduced breast tissue is usually removed in 36-48 hours.
  • The postoperative period is usually pain-free and a tight pulling feeling on the chest is to be expected. You can resume to desk work in about 10 - 14 days.
  • Physical exertion is to be avoided for a month after the surgery. Complicated physical movements can be resumed only after 6 weeks.
  • Initial follow up visits will be scheduled every couple of days. After the first week of the procedure, we need to see the patient only once a week.
  • Wearing an all-cotton sports bra after the initial healing period is recommended.
  • Underwired bras can be worn only after 6 - 8 weeks.
  • Driving, gyming or fondling of the breasts is to be avoided for 6 - 8 weeks.

Complications

    Major complications are unusual following breast reduction. Minor complications like fat necrosis delays wound healing, serous collections in the breast can happen in a small percentage of cases. These problems can be easily tackled. They do not affect the ultimate result. A decreased sensation on the nipple areola or other parts of the breast and pigmentation changes can occur following Reduction Mammoplasty. This improves naturally over a period of time. Mild asymmetry might occur which is usually not very noticeable. Nipple loss and skin loss are very unusual and rare complexities. The breast reduction technique is chosen based on the extent of breast reduction required for the breast size that you want to achieve. Normally C or D cup breast size looks good on most people.

  1. Scars around the NAC, below the breast and those joining the two (anchor or lollypop shaped) may not fade and rather look more prominent and hyperpigmented.
  2. The nipple placed too high on the breast mound may look cockeyed and difficult to correct.
  3. The nipple areola complex may be sensitive or insensate in a few people.
  4. The breast tissue once removed cannot be replaced as the reduction is permanent.

  1. You can resume most of your activities from the next day of surgery. If undertaken at the hospital, you may be discharged in 2 days. We encourage you to keep your actions restricted for at least a week.
  2. Vigorous movements of the arm may be restricted for a few weeks.
  3. Fondling of the breasts is restricted for a few weeks.
